Mammogram Compass (a service of 5D Cancer Services) is required by law to maintain the privacy of your protected health information ("PHI"), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

We may use your health information to provide, coordinate, or manage your care — for example, sharing information with your radiologist, breast surgeon, or other providers involved in evaluating your mammogram findings.

We may use your health information for our internal operations, such as quality review, staff training, and improving the services we offer.

- Right to request confidential communications. You may ask that we contact you in a specific way (for example, only by phone, or only at a certain number).
- Right to a paper copy of this notice. You may request a paper copy at any time, even if you agreed to receive it electronically.
- Right to be notified of a breach. We will notify you if a breach occurs that may have compromised the privacy or security of your information.

A Note About Email
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us by calling (435) 900-7060, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future. The current notice will always be posted on this website with its effective date.
